Prescription Review  

It is beyond that one quality of life can be improved by enhancing the standards of the medical treatment at all levels of the health care delivery system. Audit of prescription—a written order from the physician to pharmacist which contains the name of the drug, its dose and its method of dispensing and advice over consuming it—should be part of it.

At the beginning of this year, the Chief Secretary of J&K called for a prescription audit in order to streamline prescriptions of drugs by doctors. He had asked the Department to ensure that all doctors produce their seal and signature on the prescription card, in case the prescribed drugs include non-generic medicines to be procured from the market.

Prescription is an important means of therapeutic intervention by a doctor which reflects his approach towards safe prescribing. It’s a well researched fact that inappropriate use of drugs is a global health problem, especially in developing countries. Irrational prescriptions have an ill effect on health as well as health-care expenditure.

Prescription auditing is an important tool to improve the quality of prescriptions, which in turn improves the quality of health care provided.

It is important that the prescription is in line with the best practices mentioned in the World Health Organization guidelines as well as the National Accreditation Board for Hospitals and Healthcare which was founded in 2006.  In a step towards meeting this endeavor, the Health and Medical Education Department had formed a committee comprising doctors (Medical Superintendents) and Nodal Officers (doctors) to conduct prescription audits in Government Medical Colleges of Jammu and Srinagar and its Associated Hospitals. The panels had been tasked to prepare and submit fortnightly reports to the respective HODs, who shall prepare a comprehensive monthly report along with their recommendations, highlighting the actionable points, which shall be submitted to the Administrative Department. The panels, if still in practice, need to check if the medical practitioners are indulging in practices that go against medical ethics such as prescribing medicine not even required by the patient for mere promotional purposes.  It would be unfair to blame the entire community but at the same time there is no denial that promotional practices eventually lead to irrational prescriptions and pushing of high-cost drugs. One cannot ignore the complexity of the medical field. It is imperative for the administration to take appropriate measures for the holistic clinical audit and quality improvement process that seeks to improve patient care.

 

BJP retains power in 4 States, AAP wins Punjab

New Delhi: The BJP on Thursday retained power in the politically crucial state of Uttar Pradesh, Uttarakhand, Goa and Manipur while the AAP swept Punjab with three-fourths majority to give a boost to its pitch for a national role and the opposition camp ahead of the 2024 Lok Sabha polls.

As counting of votes in the Assembly elections to five states continued tonight, Holi came a week early for the BJP as an incumbent government returned to power in UP after over three decades with the saffron party riding on Prime Minister Narendra Modi’s popularity and Chief Minister Yogi Adityanath’s effective implementation of welfare policies to create a “new history”.

The spectacular rise of the Arvind Kejriwal-led Aam Aadmi Party(AAP)outside its Delhi turf with an unprecedented mandate in Punjab and a toehold in Goa by bagging two seats also foretells a big likely churn in the opposition camp. While Kejriwal hailed the party’s performance in Punjab as a “revolution, it was a hat-trick for the BJP in Goa.

“Poll results highlight people’s stamp of strong approval for BJP’s pro-poor pro-active governance,” Modi said in his address to jubilant party workers at the party headquarters in Delhi. “Those who look at UP through prism of caste insult it; people of state repeatedly voted for politics of development since 2014.”

The outcome of the Assembly polls held over February and March and seen as a barometer of people’s mood also saw the Congress’ epitaph written on the electoral battlefield.

The Congress, now in power only in Rajasthan and Chhattisgarh, an all-time low, lost Punjab and could bag only two seats in UP notwithstanding the high-decibel campaign by the Gandhi siblings Rahul and Priyanka. Like the Congress, the AAP will also be in power in two states.

In trends and results available for the 403 seats in Uttar Pradesh where Adityanath also achieved the maiden feat of an incumbent returning to power in the state after serving a full term, the BJP crossed the majority mark garnering 202 seats and was leading in 53, short of its earlier count of 312.

The BJP’s showing in UP also indicated that the devastating second Covid wave last year and the intense protests by farmers against the three contentious farm laws, which was subsequently withdrawn, did not appear to have any major impact on the fortunes of the saffron party. According to Election Commission data, the BJP won or was leading in bulk of the 113 seats in western UP where it was thought to be on a sticky wicket.

“People have buried the politics of caste and religion by ensuring the victory of BJP and its allies in Uttar Pradesh under the leadership of Prime Minister Narendra Modi,” Adityanath said at the state BJP headquarters in Lucknow where excitement was palpable with the party workers dancing to popular tunes and throwing gulal at each other.

Bulldozers — real as well as miniature versions — were at the centre stage of the celebrations at the BJP office in the state capital as they figured in the poll campaign in a reference to the action taken against criminals and what the state government calls buildings linked to the ‘mafia’. SP leader Akhilesh Yadav, a former chief minister, had called Adityanth a ‘Bulldozer baba’.

The Samajwadi Party(SP), which made a vigorous bid for power with Akhilesh Yadav attracting huge crowds at campaign rallies, won 75 seats and was leading in 36, a significant jump from the 47 last time.

Adding to the saffron party’s tally, BJP ally Apna Dal (Sonelal) won 9 seats and was ahead in three. The BSP, which barely made a campaign splash, was leading only in one seat with a total vote share of 12.7 per cent.

The BJP increased its vote share in almost all the states that went to polls including a marginal rise in Punjab, where it just managed to win two seats.

In UP, the BJP’s vote share rose to 41.8 per cent as against 39.67 per cent in 2017 assembly elections, an increase of 2.13 per cent.

The vote share of the SP increased from 21.82 per cent in 2017 to 32.02 per cent.

The BSP had a vote share of 22.23 per cent in 2017 and the Mayawati-led party won 19 seats.

The first set of Assembly elections in 2022, seen as a pointer to general elections two years away, was also the AAP’s stepping stone out of Delhi.

In the 117-member Punjab Assembly, the AAP won 92 seats, Congress 18 , SAD 3, BJP 2, BSP and Independent won one seat each. In terms of vote share, the AAP won 42 per cent of the total votes cast, as against 23.7 per cent it had won five years ago.

The strong AAP wave in Punjab saw many bigwigs losing — including SAD chief Sukhbir Singh Badal and his father Parkash Singh Badal, Congress’ Chief Minister Charanjit Singh Channi from both the seats he contested, former chief minister Amarinder Singh who left the Congress to join hands with the BJP and Punjab Congress president Navjot Singh Sidhu.

The Congress vote share slumped in Punjab from 38.5 per cent in 2017 to 23 per cent, with the party holding on to just 18 seats as against 77 seats it had won five years back.

“First this revolution happened in Delhi, then in Punjab and it will now happen all over country,” Kejriwal said at the AAP party headquarters in Delhi.

“In the coming days, AAP will become a national force…the party will emerge as the national and natural replacement of Congress,” party leader Raghav Chadha added while addressing workers at a rented accommodation of its chief ministerial candidate Bhagwant Mann in Sangrur.

From a comedian to the chief minister-designate, Mann, who has come a long way in a political career of just over a decade, said the oath-taking ceremony of the new Punjab cabinet will be held at Khatkar Kalan, the ancestral village of legendary freedom fighter Bhagat Singh in Nawanshahr district.

In the 2017 assembly polls in Punjab, the Congress had ended the SAD-BJP combine’s run by bagging 77 seats. The AAP had managed to get 20 seats, while the SAD-BJP had won 18.

“Humbly accept the people’s verdict. Best wishes to those who have won the mandate. My gratitude to all Congress workers and volunteers for their hard work and dedication. We will learn from this and keep working for the interests of the people of India,” former Congress president Rahul Gandhi said on Twitter.

Belying predictions of A fractured mandate, Goa Chief Minister Pramod Sawant has been able to deliver victory for the BJP which is coming to power for the third time.

The BJP, which bagged 20 seats, said it has received letters of support from the Maharshtrawadi Gomantak Party (2 seats) and three Independent MLAs, enabling the ruling party to cross the simple majority mark of 21.

The BJP won big in Uttarakhand getting 45 seats to retain power and was ahead in two in the 70-member Assembly even as incumbent CM Pushkar Singh Dhami lost by 6,579 votes in Khatima, a seat he had been winning for the last two consecutive elections.

Throwing a spirited challenge to the BJP as the Congress poll campaign head for the state, Harish Rawat, a former chief minister, could not pilot the party’s return to power in the state nor could he save his own seat of Lalkuwa. The Congress won 17 and was leading in two.

Assembly polls mandate has decided 2024: PM

New Delhi: Euphoric over the results of Assembly polls, Prime Minister Narendra Modi tonight declared that the mandate has “decided” the results of 2024 Lok Sabha elections.

Addressing the BJP workers at party headquarters, he said the results have shown that the days of dynastic and caste politics are numbered.

During his 40-minute address, Modi declared that the vigour to fight against corruption will be stepped up in coming times. The Prime Minister slammed “those who were trying to stonewall investigation of corruption cases and were defaming the probe agencies”.

The Prime Minister also expressed strong displeasure on the statements made by the Opposition leaders over the evacuation mission involving the war-ravaged country of Ukraine.

“Unfortunately, even when thousands of Indian students were stranded in Ukraine, attempts were being made to break the morale of the country,” he said, targeting the Opposition.

“The mandate demonstrates that people have faith in BJP’s pro-poor policies, delivery system and transparency in governance,” he said. Expressing gratitude to the voters, the Prime Minister specially thanked the “mothers, sisters, youth and the first time voters” for the favourable poll outcome.

“I am specially thankful to our mothers and daughters for extending support to the BJP. Our naari shakti believes in the policies of BJP. They feel that the BJP government looks after them well,” he said. The PM said that BJP workers worked very hard in the elections and won the trust of the people.

“In successive polls, the people of Uttar Pradesh have shown they supported the policy of development and not caste,” he said, emphasising that the days of caste politics in UP are over. The Prime Minister thanked the BJP workers for “keeping the party flag flying high, despite unfavourable conditions in Punjab.”

He also launched a veiled attack on Aam Aadmi Party. “The BJP workers in Punjab will leave no stone unturned to safeguard the state from separatist forces,” the PM said.

He also highlighted the government’s action plan in the near future.

“From here, we are going to act on two tracks simultaneously. On the one hand, our emphasis is on the welfare of villages, the poor, the small farmers and small entrepreneurs, we want to work on self-reliance by giving new opportunities to the youth,” he said. (UNI)

Shah thanks voters for reposing faith in BJP

New Delhi: Union Home Minister Amit Shah on Thursday thanked the voters of four states — Uttar Pradesh, Uttarakhand, Manipur and Goa — for reposing faith in the BJP and the Prime Minister Narendra Modi’s leadership.

In a series of tweets, Shah said, “The BJP’s grand victory in Uttar Pradesh is the victory of the unshakeable faith of village, the poor and farmers in Prime Minister Modi’s welfare for the poor. Voters have re-affirmed in Yogi Adityanath government’s fearless and corruption free governance. I thank people from the bottom of my heart for this massive victory.”

Shah also congratulated Uttar Pradesh BJP chief Swatantra Dev Singh and party workers for the grand victory and said, “BJP is committed to fulfill the expectation and aspiration of people of Uttar Pradesh.”

He also thanked the people of Uttar Pradesh for reposing faith in Prime Minister Modi’s leadership in 2014, 2017, 2019 and now in 2022.

Shah assured voters of Goa that BJP under the leadership of Prime Minister Modi will leave no stone unturned in fulfilling their aspirations. “Grateful to the people of Goa for reposing their faith in the BJP. BJP under the leadership of PM @narendramodi Ji will leave no stone unturned in fulfilling the aspirations of our sisters & brothers of Goa. Congratulations to @DrPramodPSawant, @ShetSadanand & our karyakartas,” Shah tweeted.

Thanking voters in Manipur, Shah said, “Thank you, Manipur. PM @narendramodi Ji’s resolve for a prosperous North-East has earned him a special place in the hearts of the people of our NE region, this victory is a testament to the same. Congratulations to CM@NBirenSingh Ji, @AShardaDevi Ji and our karyakartas.”

He also thanked people of Uttarakhand for giving BJP an opportunity to serve again. “Devbhoomi has reposed its faith in developmental and public welfare works under leadership of Prime Minister Modi led BJP government,” Shah said. (IANS)

Congress ‘disappointed’, CWC meet soon

New Delhi: The Congress on Thursday said it is disappointed with the results o f the polls in five states, and party president Sonia Gandhi will soon call a meeting of the Congress Working Committee to discuss the results. Party’s chief spokesperson Randeep Singh Surjewala said Congress was disappointed with the results of polls in five states, but has not lost its courage.

Addressing a press conference at the Congress headquarters, Surjewala blamed former chief minister Amarinder Singh for the party’s defeat, and also added that the results were not as per expectation.

“The results of polls in five states have been against the expectations of Congress. We were expecting better results in Goa, Uttarakhand and Punjab. We accept we failed in getting the blessings of people,” Surjewala said.

Talking about Punjab, he said, “We tried to give a polite, clean and ground based leader in the form of Charanjit Singh Channi, but we could not fight four and half years of anti-incumbency that developed in the rule of Captain Amarinder Singh. We accept the verdict and congratulate AAP, Bhagwant Mann and Arvind Kejriwal”.

He said Congress succeeded in reviving itself in Uttar Pradesh. “Even though it did not convert into seats, Congress reached every street and locality,” he said.

Surjewala also said Congress contested the elections well in Uttarakhand and Goa, but could not reach victory.

“This is a lesson that we need to work harder on the ground,” he said. Congress’ performed poorly in all five states that had gone to polls – Uttar Pradesh, Uttarakhand, Punjab, Goa and Manipur.

BJP is headed towards return to power in Uttar Pradesh, Uttarakhand, Goa, and is also leading in Manipur. In Punjab, Congress faced defeat from Aam Aadmi Party which swept the polls. (UNI)

2 LeT militants killed in Pulwama encounter: Police

Srinagar: Two militants affiliated with Lashkar-e-Toiba (LeT) were killed while two Army men sustained splinter injuries in an encounter that raged between forces and militants in South Kashmir’s Pulwama district.

A police spokesman said that based on a specific input regarding the presence of militants in village Naina, Batpora area of Pulwama, a joint cordon and search operation was launched by Police, Army (55RR) and 182Bn CRPF in the area.

“During the search operation as the presence of militants holed-up in a separate structure nearby a local masjid got ascertained, they were given ample opportunities to surrender and repeated appeals were made to them, instead, they fired indiscriminately upon the joint search party leading to an encounter,” the official said.

A police spokesman said that keeping in view the sanctity of masjid, the joint team exercised maximum restraint to ensure no damage to it and two army personnel sustained splinter injuries during the encounter.

“Both the injured were evacuated to hospital where their condition is stated to be stable,” he said. “In the ensuing encounter, two militants affiliated with LeT were killed and their bodies were recovered from the site of encounter.”

They have been identified as Shahid Ahmed Khan son of Mohi-ud-din Khan resident of Batpora, Pulwama and Fayaz Sheikh son of Gh Rasool Sheikh, resident of Shahpora, Ganderbal.

“As per police records both the killed militants were categorized militants and were wanted by law in many crime cases,” the spokesman said.

The IGP said that two local militants of Lashkar-e-Toiba first entered the masjid in Naina, Pulwama and after initial exchange of fire they took shelter in a nearby structure. “Both were killed,” he said.

Defence Spokesman in Srinagar Colonel Emron Musavi said that ssecurity forces established an initial cordon comprising 25 houses at Naina, Batpora, Pulwama.

“Systematic search commenced at 7:45 am. During the search, it was discovered that the militants had taken shelter inside a masjid. The militants were spotted to be located in the top floor of the minaret. Simultaneously civilians from nearby houses were evacuated. The security forces maintained continuous surveillance on the movement of the militants,” he said.

He said that at around 10:10 am, the militants opened indiscriminate fire and lobbed grenades from the top of the minaret onto their own troops in the cordon. “Fire was retaliated by troops who had occupied dominating positions resulting in the neutralisation of two militants. Earlier, the grenade lobbing by the militants caused splinter injuries to three soldiers,” he said.

TRF militant killed in Hazratbal shootout: IGP

Srinagar: A foreign militant was killed in a brief shootout that took place outside the revered shrine of Hazratbal, Srinagar on Thursday.

A police spokesman said that three militants came to snatch a weapon from a policeman guarding the revered shrine but the attempt was foiled in which a foreign militant was killed while two others managed to flee from the spot.

Police identified the slain as Manzoor alias Haider alias Hamzah, Pakistani national. “The timely action of small police party who were tracking them led to the elimination of foreign militant. Efforts are on to track the other two militants who fled from the spot. As per police records, slain foreigner was a close associate of slain commander Mehran of LeT (TRF) and involved in several crime cases including civilian atrocities,” the police spokesman said.

Talking to reporters outside the Hazratbal shrine, IGP Kashmir said that three militants including a foreign militant came to snatch weapon from a policeman guarding the revered shrine of Hazratbal and in a brief shoot-out, one foreign militant was been killed while two managed to flee from the spot.

“Three militants including a foreigner who was active for the past six months in Srinagar tried to snatch weapon from a guard at Hazratbal shrine. The alert policeman fired at the militant and foiled the bid. In retaliation, one foreign militant whose video was also viral on social media recently, was killed while two others managed to flee who are being tracked down,” IGP Kashmir told reporters as per a local news gathering agency.

He said militants are using religious places to take shelter but police are very active and will foil all such bids.

Missing TA soldier found dead, IGP says probe begins

Budgam: The body of a missing soldier from Central Kashmir’s Budgam district was found at Labren village of the district, police said on Thursday.

Sameer Ahmed Malla, son of Muhammad Yaqoob Malla of Lokripora, a Territorial Army soldier was posted in Jammu and had come to his home on leave. His wife Rifat had delivered her second baby last week only.

Malla had left his house on March 7 and didn’t return. His wife and parents had appealed all to allow Sameer return home for the sake of his two sons—a three-year-old and the newborn. However, on Thursday, a pall of gloom descended on his family when Sameer’s body was spotted at Labren-Dalwesh village of Khag area of the district.

Inspector General of Police (IGP) Kashmir Zone, Vijay Kumar said that the body of soldier Sameer was found. “There was no mark of firearm injury on his body. Investigation is going on. We are looking to both aspects, militancy-angle and murder,” he said.

Bank guard fired upon, rifle snatched

Pulwama: Bank guard was injured when suspected militants fired upon him in Muran Chowk of Pulwama on Thursday. The militants also fled away with his service rifle, officials said.

They said that the militant attacked the bank guard and he suffered injuries in right leg. The bank guard, Abdul Hameed Wani of Tahab, was hospitalized while entire area was immediately cordoned off by police and army, a police officer said.

“(Militants) fired upon and injured a bank guard identified as Abdul Hameed Wani of Tahab Pulwama at Murran Chowk area of Pulwama and snatched his 12 bore rifle,” a police spokesperson said in a statement issued in the evening.

“The condition of the injured is stated to be stable,” he said, adding,. The area has been cordoned off and search in the area is in progress.” He said that a case has been registered and “investigation is in progress.” (GNS)

 

Education system needs to update for future industry trends: Kansal

Jammu: Principal Secretary, Higher Education Department, Rohit Kansal presided over the ongoing Mahatma Gandhi National Fellowship academic module of Sankalp at Hotel Asia on Thursday.

The 12-days programme, from March 7 to March 19, has been conducted by the Indian Institute of Management, IIM Jammu.

MGN Fellowship is a two-year programme which combines classroom sessions at IIM with an intensive field immersion at the district level to strengthen the district skill administration and the District Skill Committees (DSCs).

Speaking at the occasion, the Principal Secretary underlined the need to reap benefits of demographic dividend of India in the form its vast young population. He stressed on the need to successfully convert it into a talented workforce through modern education.

Elaborating education sector, the Principal Secretary said that the sector needs to be updated to be more gainful and employment oriented. ‘Reorientation of our education system towards skill development is the need of the hour. We need to prepare better skilled workforce through education’, he said.

The Principal Secretary called for aligning skill and education being imparted by universities with workplace opportunities of the future. He said that the system need to upgrade itself to cater to the upcoming Industry 4.0 trends.

Rohit Kansal shared his ideas on a plethora of subjects, including doubling of farmers income, efficient delivery of public services and social welfare schemes etc. He called upon the fellows to act as crusaders of change and give recommendations to government for institutionalisation of their novel initiatives. He suggested that the fellows take inputs from local stakeholders while proposing any development for the latter.

The Principal Secretary also shared lessons from the success of Back to Village programmes and hailed its grounds-up approach.

IIM faculty would teach course content under each course module that is consistent with 40 contact days; along with off-campus mentoring, field experience, and workshops that should form an integral part of the learning. Fellows from J&K, West Bengal, Ladakh, Chandigarh and Puducherry are participating in the programme.

The programme has been designed by the initiative of Ministry of Skill Development and Entrepreneurship (MSDE), Government of India, and implemented in collaboration with State Skill Development Missions (SSDMs).
